To respect everyone's time, appointments canceled or missed with less than 48 hours' notice incur an $85.00 fee. A signed financial policy and a credit card on file are required to secure your visit. Please send your phone number, address, and insurance member and group IDs at least one business day before your appointment. Otherwise, we may need to cancel and reschedule.
